Always the Support You Need – When You Need It
Placing a loved one in a facility is often confusing, and emotionally trying for both families and residents themselves. Our social service department will provide the support for both you and your family as you transition to your new surroundings and meet our staff. We are available to answer questions, talk about feelings and address issues with other team members that you and your family may be hesitant to do.
The social service department at the Shrewsbury Nursing and Rehabilitation Center (SNRC) is responsible for scheduling care and discharge meetings. We are also responsible for providing education on community services for those returning home or to an alternative living environment and coordinating the discharge.
Social service is an essential part of the interdisciplinary team which ensures that your emotional, psychosocial and physical needs are met. The focus of the team is to provide the best quality of life for all our residents, recognizing that residents are individuals with their own specific needs, desires and goals.
For those who continue to have a difficult time with placement, whether they are here for a short term stay or long term, social services coordinates psychiatric services as agreed to by the resident and or family, and as ordered by the physician. Social service is also available to provide information regarding available insurance, and will make referrals for assistance with the Mass Health application process.
Social Service schedules periodic family/resident meetings to provide education on issues that affect those that reside at the Center. At the social service department, it is our objective to ensure that the relationship with you and our staff will be a positive one.
