Smoothing the Transition to a New Life
Short Term Rehabilitation Admissions – You may be admitted to the Shrewsbury Nursing and Rehabilitation Center (SNRC) directly from the hospital after surgery or following an illness. The first step is yours. Tell your hospital case manager you would like to come to the SNRC for your skilled nursing and rehabilitation. Our director of admissions will then meet with you and your family in the hospital and conduct a clinical evaluation to determine if SNRC can meet your needs. At this time we will also review your insurance benefits to confirm that you have adequate coverage for your stay. If everything is in order, the director will work with the hospital to facilitate a comfortable transfer to SNRC. She will also review with you and your family the type of rehabilitation care you will undergo. Upon your arrival, everyone at the SNRC will be committed to make your transition to the Center as smooth as possible.
Long Term/Respite Care Admissions – We are happy to accept new residents at the SNRC from home, an assisted living facility or from another nursing center. We will work with you, your primary care physician, The Elder Services Organization and your family to review the process of admission, which includes a tour of the Center, discussion of how to make the transition as effortless as possible and what personal requirements you might have once you move to the Center. Because of the increased complexity of the healthcare system and more stringent regulations imposed on the qualifying process, we provide assistance with filing Medicaid applications for qualified applicants. A professional who knows the process will walk you through the application, and ensure that all forms and information are correct and complete when applying for assistance.
